In preparing a petition, please consider the following:
The petition forms are available on the Judicial Council of California’s website. Each
petition type requires multiple forms and has specific instructions. Persons completing
these forms are encouraged to pay close attention to the information requested. As part
of the petition, please also submit the corresponding (with the case caption information
completed) decree or order.
One petition may be used to request name changes for multiple immediate family
members (i.e., parents and minor children). The full names of each petitioner must be
included in the caption and body of the petition. Initials are permitted when it is stated
that such initials represent the actual name.
Please also complete the following additional forms for Los Angeles County for
petitions submitted for filing:
Civil Case Cover Sheet Addendum and Statement of Location (LACIV 109)
Name Change Criminal History Assessment (LACIV 226)
The petitioner’s age and date of birth must be stated. Adults must be at least 18 years
of age. If the petitioner is under 18 years of age, the petition may be made by a parent,
near relative, guardian, or as otherwise specified.
A completed original proof of service when required should be filed with the court at
least one week prior to the hearing. When required, publish in a local newspaper of
general circulation (those newspapers legally qualified to publish orders and notices).
Allow sufficient time for the newspaper to file a Proof of Publication with the court at
least one week before the hearing.
Approximately one week after the hearing a certified copy of the decree/order issued by
the court may be purchased. The request may be made in person or by mail by
contacting the clerk’s office.
