INSTRUCTIONS FOR FILLING OF APPLICATION FORM

1. No Column to be left blank (if not applicable should be marked as NA)

2. Application Form should be submitted between 0800-1200 hours daily (except holidays) at the window of Visa-1 at the Consulate

General of Pakistan, Dubai. Applicant will not be entertained after 1200 hrs.

3. Application should be submitted by individual applying personally along with all original documents.

4. In case of children, both Parents (Mother & Father) need to give consent form/ NOC and must be physically present at time of

submission.

5. Consent form/ NOC must be signed at Visa Window. In case of single parent court custody documents of the child are also required

to be submitted alongwith the application.

6. Visa will be refused on furnishing any false information.

7. Signature wherever are required on application form must be with blue ink.

8. If applicant unable to visit the Consulate for interview after twice upon calling, his application will be rejected.

DOCUMENTS REQUIRED ALONGWITH APPLICATION FORM

1. Application form - 4 x Sets of application form with all sporting documents

2. Fresh Photographs passport size (White back ground) - 4x Passport Size and 1x small size (5x photographs).

3. Photo Copies of passport of applicant (1

& last page of passport, Aadhar Card, UAE visa and copy of Emirates ID). 4x copies of

each document

4. Photo Copies of spouse passport (1

& last page, UAE visa and Emirates ID along with CNIC ( in case of Pakistan national)

5. No Objection Certificate from sponsor in UAE (Original)

6. Invitation from Pakistan on Affidavit (Notarized) in original along with copy of CNIC and valid mobile no of invitee.

7. Marriage certificate in case of spouse is Pakistani national (duly attested from Consulate General of Pakistan Dubai).Moreover

marriage status must be updated in Nadra.

8. If applicant changed his/ her nationality, will provide citizenship certificate.

9. In case of change of religion conversion certificate must be attached.

10. If applicant is an employee, he will attach NOC from company if he is in partnership so he will provide copy of Trade Licence.

11. Birth certificate copies of children must be attached with application if their parents are applying their visa.

12. In case of Business Visa (following additional documents should also be attached):-

a. Photo copy of trade licence in case of own business and NOC in case of employee.

b. Personal bank statement of applicant for the last six months.

c. Invitation letter from chamber of commerce (in original), if applicant wishes to visit more than one city, invitation letter for each

city is required.

d. Invitation letter from the company to which individual visiting on company printed letter head.
